Click on the "Create New Form" link in the middle of the page to begin creating a new form.
Fill out the required information and then click Next.

For Dropdownlist, choose a label for your drop-down list and then type in the options for your drop-down list.
After typing each option, click "Add" to add it to the list. Then click "Save".
For Textarea, simply choose a label for your Text area and decide whether or not is required. Then click "Save".
For Radio button, choose a label for your Radio button, decide if it is a required field, and type in your choices.
After each choice, click "Add" to add it to the choice list. Then click "Save"
For Checkbox, choose a name for your Check box. Then click "Save".
As you add controls, they will appear in the preview. When you are finished adding controls, click "Activate" to finalize your form. Note that your form is no longer editable after it is activated.
